As the Patient Care Manager Assistant, you are responsible for assisting the Patient Care Manager in coordinating a team of staff to assure the continuity of high-quality care to patients assigned to the team's area. Primary Responsibilities: Ensures that the team is committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines Prepares patient list and necessary documents for weekly case conferences Provides direct patient care, as necessary, in accordance to scope of practice, plan or care and physician orders, and in coordination with other members of the health care team.
