Job Responsibilities Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care . Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management. Perform in-home patient assessments, reassessments, and provide intermittent nursing services. Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning. Supervise and provide in-service education for staff, conducting competency evaluations. Participate in quality improvement programs and policy reviews. Provide 24-hour on-call nursing coverage on a rotating basis. May assist with PCA Training Program as Needed. ***Sign on Bonus up to $10,000*** This number is based on the number of hours the individual is working on a part-time basis.
