KEY ACCOUNTABILITIES

· Provides direct clinical care and services to individuals, groups and the community. This may include nursing care and services in the areas of population and community health, basic mental health, chronic disease prevention and management, geriatrics, and other areas of nursing.
· Assesses and monitors clients actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Supports activities of daily living and where appropriate encourages client participation and increasing independence in these areas.
· Establishes and maintains a therapeutic relationship with the client and their families through the use of interpersonal and interviewing techniques ensuring client choice and autonomy in decision-making and care planning.
· Contributes to the development and ongoing changes to the individualized Recovery Plan, including attending and providing input into multidisciplinary care planning meetings. Advocates for the interest of the client.
· Acts as a role model by demonstrating empathy, trust, and respect in all interpersonal relationships while also demonstrating strong professional values as needed in collaborative decision making.
· Promotes active care coordination and efficient resource utilization by monitoring care planning, goal setting, evaluation and documentation.
· Promotes and coordinates teamwork, facilitates team decision making and problem solving processes. Plans, prioritizes and problem solves issues related to workload distribution. Provides clinical process guidance, including the support of evidence-based decision making, to assigned primary care and interdisciplinary team staff.
· Coordinates and leads the day-to-day operation of shared living sites as directed by the Manager, within the language of the collective agreements and Progressive Housing Society Policies and Procedures. As directed, prepares monthly work schedules, approves leaves and overtime, arranges for casual staff as required, and verifies payroll information.
· Oversees effective medication administration program. Administers oral, subcutaneous, and intramuscular medications including PRN medications in accordance with organizational and pharmaceutical policies and procedures. Trains and monitors staff in medication administration.
· Participates in the selection and hiring of new staff, taking into consideration the needs of the collaborative team environment and the input of the team.
· Acts as a preceptor or mentor to guide the clinical experience of assigned new nursing staff and students as applicable; provides input to the designated family physician(s), Nurse Practitioners and/or program Manger as requested on team member’s progress in achieving established standards of care.
· Contributes to the development of reference and other resource materials required to guide practice in accordance to the established quality standards.
· Evaluates employee performance including preparation and administration of performance appraisals. Identifies learning and development needs of staff and works to implement plans.
· Provides performance management including employee discipline in consultation with the Manager and Human Resources.
· Collaborates with physicians and/or Nurse Practitioners, the interdisciplinary team and other services to develop, implement and evaluate comprehensive care plans and interventions for people and their families using systematic assessment and best practices/evidence based standards involving the person and, where appropriate, the family in the context of their community.
· Participates and attends in-service and other educational programs to develop proficiency in the areas of basic mental health, chronic disease management, assisted living, wound care and other areas of primary care practice (i.e. immunization and perinatal care) required to effectively meet the health care needs of the people served by the mental health program. Identifies learning goals and maintains and updates current clinical competence and knowledge within these areas of practice. Seeks out and uses appropriate resources when presented with a situation that requires clarification, direction or confirmation in order to deliver safe and effective care.
· Maintains a safe environment for clients/family unit and staff through:
· Injury prevention.
· Knowledge of emergency procedures and regulations,
· Using skills, physical facilities, supplies and equipment safely and appropriately
· Practicing infection control procedures.
· Participates in the investigation and follow-up of incident/injury reports and client/staff suggestions and complaints related to Primary Care.
· Participates in the orientation, implementation and evaluation of Quality Improvement methodology including collection of designated data.
· Upon the direction of the Medical Health Officer, may fulfill identified roles to eliminate or decrease the risk of reportable communicable diseases or responds to a Public Health emergency.
· Contributes to the development of administrative policies and ensures that Progressive Housing Society and clinical policies and guidelines are followed.
· Assists the program Manager in the management of financial, technical and human resources through participation in program planning processes and budget monitoring. Assists the program Manager in budget implementation and in taking corrective action to maintain a balanced budget.
· Maintains in depth knowledge regarding resources and service providers in the community; fosters good relationships with these groups and establishes appropriate relationships.
· Performs nursing procedures and interventions and monitors and evaluates the effectiveness of the procedures and interventions. Procedures include head to toe assessments, nutrition management, applying and changing simple sterile and/or non-sterile dressings, taking/recording blood pressures, administering suppositories and enemas, taking/recording temperature, pulse, and respiration; records observations and reports problems and/or changes to designated nursing staff. Performs specific nursing procedures and interventions as required.
· Communicates the client’s condition and changes to the program Manager and BMHSU Case Manager or designates as required.
· Responds to client crisis situations and provides appropriate reporting and staff debriefing.
· Provides personal care to clients including assisting with bathing, dressing, care of skin and hair; changing beds; assisting with toilet needs; and overseeing exercise routines.
· Assists clients with meals and maintains intake and output records as required.
· Performs admissions, discharges, and transfers of clients according to operating policies and procedures.
· Documents and updates all relevant information about care and procedures to achieve client safety and comfort according to site-specific policy and procedure.
· Performs related clerical duties such as answering the telephone, making appointments, ordering supplies and initiating diagnostic requisitions.
· Assists in the orientation and sharing information with new staff, students, and other members of the multidisciplinary team.
· Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings, and committees.
· Maintains a safe environment for clients, significant others, and staff by adhering to established safety policies, standards, and procedures including emergency procedures and infection control guidelines. Reports unsafe conditions and malfunctioning equipment.
· Performs other related duties as assigned.

QUALIFICATIONS

· Graduation from a recognized program for Registered Nurses with three (3) years of recent, related experience; or an equivalent combination of education, training and experience.
· Current, full practicing licensure with CRNBC
· 1+ years recent related work experience with vulnerable populations
· Life experience may be considered in part for related experience
· A valid BC driver’s license, Abstract, and vehicle for work use
· First Aid, Provincial Violence Prevention, motivational interviewing and trauma-informed care an asset
