The Role: We’re seeking a Nurse Practitioner – Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, you’ll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention. Your clinical expertise and patient-focused approach will support our mission of delivering high-quality primary care in a compassionate and efficient environment. What You’ll Do: * Perform comprehensive physical examinations and health assessments. * Diagnose and manage acute and chronic illnesses. * Order, interpret, and evaluate diagnostic tests, including labs and imaging. * Develop and implement individualized treatment plans. * Prescribe medications and therapiesin accordance withstate regulations. * Provide preventive care, including immunizations and routine screenings. * Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease. * Educate patients and families on disease management, lifestyle modifications, and preventive care. * Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies. * Coordinate care with specialists and other healthcare professionals. * Document patient encounters accurately andtimelyin the electronic health record (EHR). * Participate in quality improvement initiatives and population health management. * Other relevant duties as assigned by management.
