Description

We are looking to appoint a highly motivated Nurse Associate who has a passion for delivering safe, high-quality care to children and their families in the community.
We currently have a vacancy in our Children’s Community Nursing team for an enthusiastic Nursing Associate to join our existing team.
Working as part of the Children’s Community Nursing Team you will develop skills in areas such as medication and enteral feed administration, wound management, and gastrostomy care and you will also be key in delivering high-quality patient care in the community.
The Children’s Community Specialist Nursing team provides a range of services including support to colleagues within the primary health care team, specialist hospital teams, care within the patient’s home, care co-ordination, post diagnosis and post bereavement support to families, children and young people with a wide variety of conditions. These range from those children needing acute short term interventions to children and young people with complex healthcare needs including those requiring palliative and terminal care.
To work under the direction/delegation of a Registered Nurse to provide nursing care to children and support to families on the caseload.

1. Work as an active member of the Children’s Community Specialist Nursing Team to support and promote continuity of care established for children, young people and families on the CCN caseload.
2. Deliver high quality, compassionate care under the direction of a Registered Nurse (or other registered care professional dependent on setting) with a focus on promoting health, independence, comfort, dignity, privacy and quality of life for patients.
3. Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual.
4. Work with a mentor to take responsibility for developing own clinical competence, leadership and reflective practice skills within the workplace.
5. Provide skilled paediatric nursing care and clinical interventions to children and young people within their own home, or other applicable venues such as outpatient clinics, to ensure families are supported in the care of their children at home under the direction of a Registered Children’s Community Nurse.
6. Develop, the ability to work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed.
7. Complete accurate client records in line with CPFT policy. Be aware of the requirements of both paper and electronic record keeping on SystmOne and other external health care systems.
8. Maintain excellent communication with the child and family on the caseload and act as their advocate to ensure that they have ease of contact with the team, appropriate information and support. Records should reflect the child’s care, present condition and progress and be available to colleagues.
9. Competently deliver cares reflecting specialist evidence based practice within the community setting as required under the direction of the CCN team, e.g. wound management/enteral feeding/ tracheostomy management/ gastrostomy site care/ oxygen dependency/ technology dependent children.
10. Provide skilled, evidence based, sensitive care to children and young people who require end of life care. Liaise closely with CCN team members and other agencies to ensure good care assessment and delivery allowing the child, young person and family choices at the end of life stage.
11. Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management and evaluation of care.
12. Perform and record clinical observations and undertake clinical tasks.
13. Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Children’s Community Nurse or other registered care professionals as appropriate.
14. Report back and share information with the registered CCN on the condition, behaviour, activity and responses of individuals on the CCSN Caseload.
15. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals.
16. Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being administered.
17. Assist in the implementation of appropriate action to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers.
18. Assist in the delivery of complex care as prescribed by the registered nurse.
19. Develop understanding of caring for individuals with particular conditions for example dementia, mental illness, learning disabilities.
20. Develop skills in relation to coaching/teaching individuals/carers/other staff.
21. Assist with the implementation and monitoring of clinical standards and outcomes.
22. Develop a working knowledge of other providers’ resources and referral systems to ensure individual’s needs are met, within parameters of practice.
23. Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given.
24. Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
