JOB SUMMARY:

Collaborates with the health care team to coordinate patient and family interventions across the continuum of care, removing barriers and promoting efficient and effective use of resources within the scope of licensure. Works closely and in collaboration with the health care team to facilitate patient and family interventions by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize the patient’s health care across the care continuum. Serves in an advocacy role on behalf of patients, families, and caregivers to ensure safe healthcare.

KEY RESPONSIBILITIES

• Assesses and identifies patients with complex clinical needs that require a coordinated plan of care.
• Develops case management plan that includes strategies or alternative interventions required to attain optimal patient and family specific outcomes. May include gaps in care and minimizing barriers to care.
• Analyzes progress on the plan of care, identifies variances, and intervenes utilizing process improvement methodologies to remove barriers and progress care.
• Assists patients and families in establishing short- and long-term patient specific goals and provides support and encouragement to reach goals.
• Obtain authorization for patient services, such as prior authorizations for prescriptions, DME orders, forms, etc.
• Help patients and families understand appropriate use of emergency rooms and follow up care after an ED visit or hospitalization
• Demonstrates good verbal and written communication skills; documents and reports clearly and concisely in medical record.
• Demonstrates effective listening skills; and the ability to exercise independent judgement and critical thinking.Ability to organize and prioritize workload.
