We are seeking a dedicated Occupational Therapist to join our Housing Adaptations Team, working collaboratively across housing and social care to deliver practical solutions that make a real difference to people’s lives. You will be responsible for undertaking person-centred functional assessments in people’s homes and recommending appropriate housing adaptations to promote independence, safety and wellbeing. This includes both minor and major adaptations funded through Disabled Facilities Grants and other local authority funding streams. You will also be responsible for completing housing needs assessment to provide recommendations to support housing decisions ensuring individuals are matched to accommodation that meets their long-term needs.
