Addiction and Mental Health Worker, Middlesex Accommodation Program (MAP) OPSEU - Direct Service Full-Time, Contract (Ending March 31, 2027) This posting is the result of an upcoming vacancy Strathroy, Ontario (Middlesex Zone) - Application Deadline: March 31, 2026 - The Supportive Housing- Addictions & Mental health Worker will perform duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of individuals in the Middlesex Accommodation Program (MAP) and work with other Supportive Housing team members in Strathroy and Middlesex to support the needs of clients within our continuum of Supportive Housing. This staff will support individuals who experience persistent and chronic homelessness with a focus on supporting them to achieve housing stability. This role works collaboratively with the Middlesex County Library Community Navigator and the Middlesex Accommodation Program (MAP). Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Support individuals with physical and mental health and addictions concerns, liaise with clinical staff Support and bring awareness to recreational community outings and activities to enhance individuals’ quality of life and further connect individuals to their community Assist individuals to successfully remain in the program, providing prevention and diversion strategies to avoid homelessness Works in collaboration with the greater Supportive Housing Team that supports the presenting needs of clients in Strathroy and the county of Middlesex Assessment and Goal Planning Assess individuals’ strengths, abilities, needs and preferences Assess social and environmental factors impacting the individual, and any vulnerability and risk factors before admission into the program. Include and encourage individuals’ natural supports (e.g. family, significant friends) in goal planning, clarifying the roles each plays towards each individual’s rehabilitation as appropriate Regularly solicit feedback from the individual-served and relevant community supports regarding the individual’s mental health and progress towards his/her goals Communication and Decision Making Make decisions both independently and collaboratively Complete required individual documentation as per CMHA TV standards Complete other required documentation (e.g. monthly time sheets, vacation requests, travel claims, expense claims) within the required time frames Provide input into decisions with the Manager concerning operations of the program Ensure effective communication with Director, Managers, Team Lead.
