Job Description

As the Program Director of Integrated Care, you will play a crucial role in developing and maintaining key relationships with our clinical and operational health system partners. Your primary objective will be to contribute to the implementation and sustainability of integrated care programs that drive positive health system change.

• Lead operational planning and implementation of multiple concurrent integrated care programs
• Define service delivery requirements at the local level, collaborating with various external partners and patient populations
• Promote and implement the “One Team” approach, fostering collaboration and synergy across the organization
• Ensure smooth program operations and continuously improve processes
• Serve as the primary representative for integrated care programs, presenting to stakeholders and promoting program initiatives
• Analyze program metrics and utilize findings to drive positive change
• Mentor and support staff to maximize their potential and enhance team performance
• Stay informed of industry trends and best practices to maintain program excellence

QUALIFICATIONS

• A degree in a relevant health-related field
• Active designation as a Regulated Health Professional and current membership in good standing with the relevant professional college or association
• A minimum of 5-7 years of leadership and management experience in complex healthcare organizations
• Demonstrated expertise in healthcare operations, change management, and program development, particularly in environments involving multiple stakeholders and complex organizational structures.
• Comprehensive knowledge of Ontario’s health system design and funding models
• A track record of successful implementation of integrated care programs and initiatives
