Position Summary

The Care Coordinator provides high‑quality, client‑centred case management to individuals living with complex medical and mental health needs. Using an intensive case management approach, the Care Coordinator works collaboratively with clients, their personal supports, and internal and external care providers to create and implement coordinated care plans rooted in the client’s priorities.

The Care Coordinator plays a key role in improving clients’ experiences within the health and social service systems through advocacy, creative problem‑solving, and accountability.

Key Responsibilities

  • Manage a caseload of clients with complex needs in collaboration with internal and external health and social service providers
  • Develop and implement integrated care plans that coordinate services within Wabano and across partner organizations
  • Facilitate case conferences with clients and their support teams to enhance communication and continuity of care
  • Advocate for clients to improve their experiences navigating health and social service systems
  • Provide education and coaching to clients and their supports, including chronic disease self‑management, coping strategies, communication skills, and other life skills
  • Accompany clients to appointments, when needed, to ensure their voices are heard and they receive appropriate, high‑quality care
  • Monitor progress and adjust care plans based on clients’ goals, needs, and strengths
