Job Description About the role: In this role, you will manage a caseload of community-based elderly individuals in the Sutherland Shire and Southeast Sydney area. You will assess their needs, develop personalised care plans and manage budgets in accordance with program guidelines, to ensure each customer's unique requirements are met. You'll also onboard new customers and proactively manage existing ones with a person-centred approach. Collaboration with our Clinical, Scheduling and Service Delivery teams will be crucial to delivering comprehensive care. This is a full-time role, and you will have the flexibility to manage your own schedule each week, while working autonomously from home, our Bella Vista office and within the community. As you will regularly visit customers in their homes, you will need to be based close to, and willing to travel, within this region.
