What you'll do day-to-day: Reach out to your assigned patients on a structured schedule — some weekly, some monthly — based on their condition and risk level Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction Help patients navigate real-world barriers: late refills, insurance questions, transportation, specialist coordination, copay assistance Document patient interactions using AI-assisted tools (less typing, more connecting) Escalate clinical concerns to an on-team pharmacist using clear protocols Assist to enroll newly eligible patients into the program Why this work matters Chronic disease is won or lost between appointments.
