QUALIFICATIONS:

• Bachelor’s degree in Healthcare Administration, Nursing, Social Work, or related field (preferred).
• Previous experience in patient relations, customer service, or a similar role in healthcare (home healthcare experience is a plus).
• Strong communication, interpersonal, and conflict resolution skills.
• Ability to work in a fast-paced, patient-centered environment.
• Proficiency in Microsoft Office and patient management systems.
• Bilingual abilities (optional but preferred depending on patient population).

• Serve as the primary point of contact for patient and family communications regarding care and services.
• Manage patient onboarding process, including initial assessments, documentation, and care coordination.
• Address and resolve patient complaints, concerns, and feedback in a timely and empathetic manner.
• Maintain accurate records of patient interactions, complaints, resolutions, and follow-ups.
• Coordinate with clinical teams to ensure timely delivery of services and continuity of care.
• Educate patients and families on care plans, service processes, and patient rights.
• Monitor patient satisfaction through surveys and follow-up calls, and report findings to management.
• Collaborate with administrative and operational staff to improve service delivery.
• Ensure compliance with healthcare regulations, patient confidentiality policies, and company procedures.
