You are often the most present member of the care team — the one who helps a patient feel seen, safe, and supported every day. Key Responsibilities Include: Following the Plan of Care – Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences. Assisting with mobility and safety – Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment. Observing and reporting – Noticing and promptly reporting any changes in the patient’s condition or behavior to the RN, supporting proactive care. Practicing infection control – Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
