PURPOSE: The Community Connections Supervisor directly supervises and supports the Community Connection Case Managers. This position ensures case management services are being provided to the highest standard within agency, state, and federal guidelines. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Supervises Community Connection Case Managers by developing internal monitoring, weekly supervision, and evaluation processes. Ensures effective and consistent communication occurs between the Community Connection Case Managers and all parties of the case and identified stakeholders, including medical, educational and mental health providers. Ensures Community Connection Case Managers conduct visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being. Ensures Community Connection Case Managers meet the requirements and service standards when facilitating delivery of services in collaboration with families and youth that adhere to agency, state, and federal policies/procedures Provides direct case management to OCC referrals when needed, as determined by the Child and Family Services Leadership team. Provides supervision of Intact Family Case Managers and/or Intact cases during times of overflow or staffing needs, as identified by the Child and Family Services Leadership team. Performs supervisory responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
