Position Summary The ASA Case Manager is responsible for providing person-centered case coordination for individuals with developmental and intellectual disabilities to promote a high quality of life. Ariel Clinical Services Mission To provide safe, nurturing, and supportive environments for our clients so that they may have the opportunities to develop their strengths, maximize their potentials and fully participate in society. Essential Duties and Responsibilities 1. Establish, build, and maintain rapport with individuals, independent contractors, and other members of the multidisciplinary team in order that optimal information gathering, and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of individuals, is driven by individual choice and promotes community inclusion. 2. Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team. 3. Develop, monitor, and document mandated services and programming, and ensure individual satisfaction. Ensure individual files are audit ready and in compliance with Volume 8 rules and regulations. 4. Understand and participate in all relevant assessments to ensure appropriate level of care for individuals. 5. Facilitate home visits and meetings to assess the needs and progress of individuals and intervene as appropriate. Provide mediation as needed. 6. In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel supervisors and collaborating agencies. 7. Participate in mandatory rotation for “on call” to support our individuals. 8. Participate in ongoing program evaluation to ensure program effectiveness. Additional Duties and Responsibilities 1. This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to 2 hours without a break. 2. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position. 3. This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds. 4.
