This role manages the intake and admission process for new patients, conducts comprehensive assessments, and develops timely, individualized care plans. The RN provides both in-person and telephonic triage to evaluate patient needs, determine appropriate levels of care, and ensure effective coordination with families, healthcare providers, and community partners. In addition to triage and intake responsibilities, this role delivers skilled in-home hospice nursing care, including symptom and pain management, patient and caregiver education, and emotional support for individuals facing end-of-life challenges. The RN collaborates closely with the interdisciplinary team to ensure high-quality, patient-centered care in homes and facilities. This position also supports relationship-building with referral sources and participates in community outreach to enhance awareness and accessibility of hospice and palliative care services. This multifaceted role requires strong clinical judgment, adaptability, and compassionate communication to support patients and families throughout their end-of-life journey. Requirements Minimum Qualifications: 1. Valid and unrestricted Registered Nurse (RN) License in the State of Washington. 2. Available to work occasional evenings, nights, weekends, holidays, and On-call. 3. Demonstrated ability to adapt to different work environments and provide effective care in diverse settings. 4. Excellent assessment, critical thinking, and problem-solving skills. 5. Strong communication and interpersonal abilities to establish rapport with patients, families, and healthcare professionals. 6.
