We are seeking an experienced, dedicated and compassionate Band 6 Registered Children’s Nurse to join our Children’s Continuing Health Care Team as a Nurse Assessor, or alternatively someone who is keen to develop their skills in this area joining as a B5 development post.
In this specialist role, you will work within the framework of the NMC scope of professional Practice, delivering high-quality, family centred care assessments for children and young people with complex and continuing health care needs.
You will work in close collaboration with families, health professionals, education providers and social care teams undertaking assessment and reassessment of children with complex and long-term health needs, including those who are dependent on technology for survival, in line with the National Framework for Children and Young People’s Continuing Health Care (CHC).
You will present the assessments at the Complex Case panel so that decisions can be made regarding packages of care. Your work will help ensure that children will receive the appropriate level of care and support to meet their individual needs.
You will work closely with the lead Continuing Care assessor and Complex care lead to deliver service improvements and positive outcomes for children and young people with complex care needs.
If joining as a B5 in the development post you will be supported to gain skills and knowledge in order to meet the requirements of the B6 role.
