We are seeking an individual with strong experience in public health programs and partnerships for the Project Coordinator position. The Project Coordinator will be responsible for supporting CDIP’s work on the Philadelphia Innovative Cardiovascular Health Program (Cardiovascular Health), funded by the CDC to improve health outcomes of Philadelphians with hypertension, high cholesterol, and cardiovascular disease, while addressing social drivers of chronic diseases. The Project Coordinator will provide technical assistance to all contracted partners (healthcare practices and non-profits) implementing innovative programs to meet grant objectives and metrics. The employee in this position will also be responsible for supporting other projects within the Health Partnerships program area, including but not limited to asthma and its related risk factors, promoting access to primary care services, and strengthening resource-referral partnerships through cross-sector coalitions. The Project Coordinator may also work on other projects dedicated to addressing factors contributing to chronic disease and injury. This position reports to the Health Partnerships Program Manager and will work closely with the Health Partnerships team and members of the CDIP Data team. This position is contingent on continued grant funding. JOB SPECIFICATIONS Responsibilities/Duties The Project Coordinator will accomplish the following: Attend and actively participate in meetings with grant partners, the CDC, and CDIP, and assist with meeting coordination, preparation of meeting agendas, and meeting minutes. Work with the Health Partnerships Program Manager and partner organizations to assess progress on overall hypertension control and process measures related to achieving target metrics. Research innovative strategies for and provide technical assistance to healthcare practice partners to improve hypertension and high blood cholesterol control among their patient populations. Examples include: Supporting work to assess options for integrating tools for population health management and/or other technology. Supporting efforts to integrate screening for social and economic needs and to implement bidirectional referral systems with community resources. Lead all grant progress reporting for CDC. Support evaluation activities, which could include conducting interviews, informing evaluation design, and assisting with the development of data collection tools, for example. Research culturally informed, evidence-based interventions and policies to address social drivers of health impacting chronic disease and injury prevention. Develop and coordinate new strategies and partnerships related to asthma prevention and control, including conducting background research on care coordination, housing, air quality, and prescription coverage in Philadelphia and nationally; organizing timelines and activities; monitoring and reporting on progress; assisting with meeting coordination, preparation, and follow-up for the PDPH-convened Asthma Strategy Advisory Group; and ensuring internal and external communication. Research, identify, and support the application process for funding opportunities to advance and expand CDIP’s work as it relates to cardiovascular health, asthma, primary care access, and social drivers of health. Provide programmatic support for CDIP’s Primary Care Finder tool and work with the Data team on updating and promoting the tool. Produce presentations, reports, and media content. Identify opportunities for and facilitate collaborations within CDIP and with external organizations. Represent CDIP in coalitions and working groups, where appropriate. Possible opportunity to take on additional project management responsibilities, such as managing budgets and contracts with partners. Document and organize work processes and products.
