JOB OVERVIEW
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in various nursing specialties, including critical care, emergency medicine, and geriatrics. This role involves coordinating patient care, conducting assessments, and ensuring effective discharge planning while adhering to HIPAA regulations and utilizing medical terminology. The Registered Nurse Case Manager will play a vital role in improving patient outcomes through comprehensive case management.
Must have at least 1 year of Hospice experience within the last 3 years
HCHB charting experience is required
$2,000/Wk plus .42 a mile for patient related travel
