Description

A new and exciting opportunity has arisen with the Royal Derby Hospital’s (RDH’s) Mental Health Liaison Team for a Band 4 Rough Sleeper Support Worker to work with the team targeting individuals with homelessness and related social care issues secondary to Drug and Alcohol difficulties.
You will be working alongside experienced RMNs, RGNs, Psychiatrists and other mental health professionals supporting patients with Drug and Alcohol difficulties which not only impact upon their health but upon their housing and welfare needs.
We will support you to develop a knowledge of Drug and Alcohol difficulties, related health concerns, and how such conditions impact upon the person’s broader life.
We are looking for an individual with compassion, initiative, non-judgemental unconditional positive regard, a positive and caring approach to their work, a desire to help people be their best selves and a strong sense to collaborate with people.
We offer you the opportunity to work with people from all organisations and walks of life, gain experience in the general hospital setting as well as the community setting, with ongoing regular clinical and managerial supervision, innovative training and a network of highly motivated people.
We would welcome informal visits with prior arrangement to meet our team.
The aim of this position is to reduce the burden of disease and untimely deaths in this vulnerable and complex patient group.
You will work with service users as the RDH with a wide range of complex needs, such as homelessness, substance misuse problems, self-neglect, domestic abuse, significant risk histories and social exclusion, and who at times may present with challenging behaviours.
You will work flexibly, creatively and effectively in the role, with a high degree of self-direction and using excellent time-management skills to manage your own time and workload, under the supervision and guidance of their Ward Manager.
You will develop strong working links with professionals engaged in support of service users with such issues, such as the multi-agency rough sleepers hub (MARSH), the Rehousing Engagement Support Team (REST), Derby homes, Centenary house, Milestone house, Safespace, the Homeless Paramedic and the Homeless MDT.
You will work with the MHLT Service Manager and the Clinical Lead for Alcohol and Substance Misuse to ensure high standards and high-quality clinical care to the client group.
Join ‘Team Derbyshire Healthcare’ and become part of a talented, compassionate and enthusiastic workforce committed to a vision of ‘making a positive difference in people’s lives’.
CQC rated us as ‘GOOD’ overall, commenting on how our colleagues “treated patients with compassion and kindness” and “felt positive and proud about working for the trust.”
