ABOUT THE ROLE:

We are currently hiring a Community Health Representative who will serve as the local face of our services in the Burnaby and New Westminster areas. This role is ideal for someone who enjoys building relationships in person, is comfortable with outreach and follow-up, and has a knack for both sales and service.
This is not a remote or call-centre position — it’s a people-first, community-based role that involves meeting face-to-face with potential referral partners (such as doctors, social workers, pharmacists, and care home staff etc) and conducting in-home assessments with potential clients.

KEY RESPONSIBILITIES:

• Build relationships with healthcare professionals, community leaders, and businesses that may refer clients to us.
• Represent Just Like Family at local events, health fairs, and community meetings.
• Meet with prospective clients and their families to conduct assessments and explain our services.
• Develop personalized care plans and guide clients through the onboarding process.
• Maintain accurate and timely records in our CRM system (we use Monday.com).
• Meet weekly/monthly goals related to outreach, referrals, and assessments.
• Collaborate regularly with the internal care team to ensure a smooth client experience.
