Als casemanager dementie begeleid en ondersteun je cliënten met dementie en hun naasten gedurende het gehele ziekteproces. Je bent hun vaste aanspreekpunt, coördineert de zorg en je bent het verbindende element met huisartsen, specialisten ouderengeneeskunde en andere zorgprofessionals. Jouw doel is om de kwaliteit van leven van de cliënt te verbeteren en naasten te ontlasten.
Deze functie binnen onze organisatie is volop in ontwikkeling, waarbij jij verder gaat helpen met bouwen en positioneren. Je valt onder het thuiszorgteam, waarbij je onderdeel bent van een team van casemanagers en wijkverpleegkundigen. Het team is hecht, er zijn korte lijnen en er is veel ruimte om mee te denken en eigen regie te pakken. Onze organisatie heeft veel ervaring met dementie en heeft een groot netwerk in de thuiszorg, maar biedt ook zorg en welzijn vanuit zeven verpleeghuizen.
Wat ga je doen als casemanager dementie?
Je begeleidt en adviseert cliënten en hun naasten gedurende het ziekteproces
Je stelt zorgplannen op en bewaakt deze in samenwerking met diverse disciplines
Je coördineert zorg- en hulpverlening in samenwerking met collega’s en externe partners
Je signaleert ontwikkelingen in de gezondheidssituatie en speelt hier proactief op in
Je ondersteunt en informeert naasten om overbelasting te voorkomen
Je neemt deel aan multidisciplinaire overleggen en draagt bij aan beleidsontwikkeling op het gebied van dementiezorg
