Wadja Aboriginal Family Place is seeking a dedicated Case Manager to provide culturally safe and holistic care within a tertiary hospital setting. This is an Aboriginal Identified Position (as per s12 of the Equal Opportunity Act 2010 (Vic)). The Wadja Case Manager plays a vital role in advocating for and supporting Aboriginal families, ensuring care is delivered in a way that respects cultural values and promotes health and well-being.   You’ll be part of a team committed to improving health outcomes for Aboriginal communities, working in a supportive environment that values cultural safety and collaboration.   Classification for this position will range from Community Development Worker Class 2B Year 1 – Year 6 (XX7 – XX12) (FTE base salary $83,434 to $96,652.40 per annum per annum, plus superannuation).   What you’ll achieve: Provide culturally safe, holistic care: Assess and respond to Aboriginal patients’ social, cultural, emotional, and medical needs, ensuring care is respectful and inclusive. Coordinate complex care and discharge planning: Manage the social and cultural care of Aboriginal inpatients and outpatients with complex needs and develop culturally appropriate discharge plans in collaboration with patients, families, and multidisciplinary teams. Advocacy and informed decision-making: Support patients and families in medical consent and decision-making, ensuring they fully understand treatment processes, procedures, and rights. Facilitate access to services and supports: Connect patients and families to internal and external programs and services identified in care and discharge plans, promoting continuity of care beyond the hospital setting.
