This position is established to support case managers by implementing authorized changes, services and contacts for PASSPORT and Assisted Living Waiver individuals. Details: Conducts phone calls with individuals following hospital/nursing home discharge to complete care transition activity. Mails zone management tool to individuals following discharge with qualifying diagnosis. Assists the case manager by providing individual/informal support information to assist in gaining access to any necessary services regardless of funding source which enables individuals to remain in their community living arrangement. Accurately and timely completes changes to the amount, scope, and duration of waiver services at the direction of the case manager. Documents emails or voice mails from service providers in case notes and make corresponding adjustments to the service plan. Contacts service providers or individuals/informal support to monitor service delivery and resolve service provision problems. Completes service suspends in collaboration with case manager. Conducts timely home visits, Level of Care and Comprehensive assessments as needed, according to program specifications. Accurately maintains all appropriate documentation in formats and time frames prescribed by the PAA and the Ohio Dept. of Aging. Utilizes electronic tools and software to complete tasks.
