MD/DO from an accredited University and pending/active State Medical Board license or Medical Board License Eligible. Board Certified / Board Eligible.
· Licensed to practice medicine in the state of Louisiana
Must have current state and federal DEA certification
· Current Certification in Basic Life Support for healthcare providers, Advanced Cardiac Life support may also be required based on the scope of services
