The Facilitator ensures that the Wraparound process is implemented with fidelity and integrity, consistently maintaining hope that each family can achieve a better future. This role is responsible for initiating engagement with parents and youth, forming the Child and Family Team (CFT), developing safety plans, and creating an individualized CFT plan based on family strengths, needs, and preferences. Throughout the process, the Facilitator maintains a non-judgmental, strengths-based, and culturally responsive approach. They coordinate the development and ongoing revision of CFT plans, reflecting lessons learned from what has and has not worked. From the beginning of care, they also plan for transitions, ensuring smooth movement toward independence and stability. The Facilitator provides leadership to the team, consistently demonstrating Wraparound values and principles. They engage others in a shared mission, display strong interpersonal skills, and effectively navigate the complex systems of public services. They also build strong connections among CFT members, valuing diverse perspectives that support family-driven success. Additionally, the Facilitator provides direct services and interventions aligned with the Wraparound plan. PROGRAM DESCRIPTION: The Adoption Assistance Program serves adopted children/youth and their families. This program provides a collaborative and highly individualized process for developing specific resources and services for the referred child and their families. The goal of the program is to utilize an adoption-competent, strength-based approach to maximize the capacity of each family to meet the child’s needs, and maintain permanency outcomes and well-being. AAP uses the CA High-Fidelity Wraparound (HFW) model, which is a team-based evidence-based practice that includes an “anything necessary” approach to care for youth and families with the most intensive mental or behavioral health challenges. Requirements GENERAL REQUIREMENTS: Education/Experience Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider. Eligible activities may be claimed under Medi-Cal if all state and federal Medicaid requirements are met. Required Training - Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training, accessible through Regional Training Academies, the UC Davis Resource Center for Family-Focused Practice (RCFFP), or CACWT Licensure, Registration, Certification Valid CA driver’s license and proof of automobile insurance. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment. JOB DUTIES & RESPONSIBILITIES: Actively participate in the engagement process by providing reassurance that the family’s circumstances and support the team in identifying and celebrating achievements, growth, and strengths throughout the Wraparound process. During Team Meetings (Child and Family Team Meetings/Wraparound Team Meetings) Schedule, convene, and facilitate Wraparound team meetings, ensuring to invite natural and formal supports, guide the family’s team through meaningful participation Guide the Wraparound team through needs discovery, strengths identi?cation, and family vision articulation Lead development of Wraparound team agreements, team mission, the family vision statement, and development of the family’s goals Prioritize needs and translate them into measurable outcomes in the Plan of Care Coordinate the development of crisis and safety plans that re?ect family-driven, proactive, and reactive strategies Update and review plans ensuring all team members are aligned and aware of responsibilities Outside of Team Meetings Maintain ongoing communication and follow-up with all Wraparound and Child and Family Team members to ensure clear communication, accountability, and service delivery Communicate with the Child and Family Team (CFT) including accessing the family’s IP- CANS ratings to incorporate into planning and to provide ongoing updates as well as receive emergent information from the CFT Engage additional natural supports and professionals to expand the Child and Family team’s capacity Support transition planning from the onset, guiding the family through benchmarks toward sustainable support and success Incorporate tools such as the IP-CANS to inform planning and monitor progress JOB SPECIFIC COMPETENCIES Service Delivery Meeting expectations in Service Delivery Skills encompass the ability of paraprofessional staff to identify and recommend appropriate service types that align with the needs of clients, while staying within the bounds of their training, expertise, and professional role. This competency requires an understanding of the range of services available within the organization and the ability to match those services to the specific needs and preferences of clients. Crisis Intervention and Safety Planning Meeting expectations in Crisis and Safety Planning encompasses handling crisis situations effectively, including suicidal ideation, self-harm, and acute psychiatric symptoms, by adhering to established safety protocols and accessing appropriate resources. Participate as needed in the implementation of safety plans in collaboration with clients, their support systems, and the treatment team. Productivity & Service Billing Meeting expectations in Productivity and Service Billing skills focus on the ability to hit program productivity expectations on a monthly basis. PHYSICAL REQUIREMENTS: Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed. Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field. Required to occasionally lift and carry 10-20 pounds. Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
