Descripción del empleo Objetivo del Puesto El Técnico en Búsqueda Activa de Casos y Vinculación (TBACV) en los Sitios VICITS es responsable de fortalecer la cobertura de atención, vinculando a las personas que requieren pruebas de VIH con servicios de cuidado y tratamiento en las Unidades de Atención Integral. Implementa estrategias como la Notificación Asistida de Contactos (NAC) y la Prueba Ofertada por el Proveedor (PITC), asegurando una atención oportuna en adultos y niños. El rol requiere comunicación persuasiva y orientación a resultados, logrando motivar a las personas a realizarse la prueba, iniciar tratamiento y notificar a sus contactos, siempre con empatía, confidencialidad y ética. Además, debe generar confianza, manejar objeciones con sensibilidad y mantener relaciones respetuosas con el equipo y los participantes del proyecto, cumpliendo con las funciones del puesto. Funciones y actividades: 1. Apoyar a los servicios de pruebas de VIH y consejería en todos los usuarios que visitan las unidades de salud. Actividades: Realizar abordajes cara a cara dentro de la unidad de salud en el horario establecido. Realiza sesiones de consejería individual o grupal explicando el proceso y la importancia de la prueba. Verifica con el personal de laboratorio de la unidad de salud que se cuenten con los insumos necesarios para la realización de las pruebas de VIH. En conjunto con el personal de salud, realiza requerimientos de insumos a VIHCA, según sean necesarios para la realización de las pruebas de VIH. 2. Acompañar a las personas diagnosticadas con VIH en su vinculación a los servicios de atención y tratamiento. Actividad: Coordinar y acompañar al usuario a un máximo de tres citas consecutivas en la Unidad de Atención Integral (UAI). 3. Participar en actividades institucionales relacionadas con el tamizaje del VIH. Actividad: Colaborar y participa en las acciones planificadas por la unidad de salud para la oferta de las pruebas del VIH. 4. Asiste a reuniones de trabajo para evaluar estrategias y desempeño en servicios de VIH. Actividad: Presenta informes de resultados y compartir experiencias en reuniones organizadas por la unidad de salud o el equipo nacional o regional de VIHCA. 5. Contacta a las personas vía telefónica para su seguimiento en el proceso de diagnóstico y vinculación. Actividad: Realizar llamadas para recordar citas y brindar información sobre el acceso a los servicios de tratamiento. 6. Informa a los usuarios sobre el proceso de vinculación y acompañamiento en la UAI. Actividad: Explica de manera clara y detallada el procedimiento de vinculación, tiempos estimados y derechos del usuario. 7. Aplica el consentimiento informado a las personas que acepten los servicios de pruebas de VIH. Actividad: Explica el documento de consentimiento y obtener la firma de los participantes de manera voluntaria. 8. Proporciona información sobre el procedimiento de la prueba de VIH según el algoritmo nacional para prueba diagnosticas del VIH. Actividad: Detalla los pasos de la prueba, tiempos de entrega de resultados y acciones a seguir en caso de diagnóstico positivo. 9. Resuelve dudas de los participantes sobre el VIH y el proceso de vinculación las Unidades de Atención Integral del país. Actividad: Brinda respuestas claras y fundamentadas a las inquietudes del usuario mediante sesiones informativas o durante los procesos de consejería. 10. Coordina el acompañamiento de los usuarios VIH positivos a la UAI de su elección. Actividad: Gestiona la logística del traslado o asistencia a la unidad, considerando la disponibilidad del usuario y programación de citas brindada por la UAI. 11. Registrar la información de los usuarios vinculados a las UAIs. Actividades: Completa formatos autorizados con datos como fecha de diagnóstico, código de identificación, CD4, Carga Viral basales y cualquier otro dato requerido según el formato proporcionado por VIHCA. Notifica el mismo día a la supervisión inmediata de VIHCA, los procesos de vinculación realizados. 12. Colabora con las UAI en la mejora del estado emocional y adherencia al tratamiento de los usuarios. Actividad: Coordina sesiones individuales de apoyo emocional y estrategias para fomentar la adherencia a la terapia antirretroviral. 13. Brindar soporte emocional antes, durante y después del diagnóstico para facilitar la vinculación a la UAI. Actividad: Ofrecer acompañamiento psicológico y motivacional a los usuarios en su proceso de adaptación al diagnóstico. 14. Aplicar la estrategia de Notificación Asistida a Contactos (NAC) en conjunto con el personal de salud. Actividades: Realizar entrevistas con los usuarios VIH positivos para explicar la estrategia y solicitar su consentimiento en la aplicación de la estrategia y la obtención de contactos. Determina en conjunto con el caso índice, la modalidad de oferta de la prueba de VIH para alcanzar a los contactos. Priorizar la oferta de la prueba de VIH en la unidad de salud y en casos excepcionales, oferta otra forma de tamizaje como la prueba de VIH en la comunidad o bien, en donde esté disponible la autoprueba del VIH. Lleva un registro riguroso de la información de cada contacto obtenido., así como de los seguimientos realizados para la oferta de la prueba de VIH según los formatos proporcionados para esta estrategia. Registra en tiempo y forma la información relacionada con esta estrategia en la plataforma REDCap. Realiza en conjunto con el personal de salud de la unidad el monitoreo y desempeño de esta estrategia mediante la cascada de caso índice. 15. Informar a las UAI sobre la participación de los usuarios en la estrategia NAC. Actividad: Reportar a la UAI a través del formulario correspondiente si el usuario ha aceptado o rechazado la estrategia, garantizando la confidencialidad y el seguimiento. 16. Implementa estrategias adicionales de oferta de pruebas, como Pruebas Iniciadas por el Proveedor de Salud. Actividad: apoya con el equipo de salud la identificación y acompañamiento del tamizaje de los usuarios que visitan la unidad de salud. 17. Elaborar planes, reportes y actividades administrativas de acuerdo con requerimientos del equipo de VIHCA y del Ministerio de Salud: Actividades: Realiza planificación y ejecución de sus actividades, según periodicidad definida por la supervisión inmediata del tamizaje y/o diagnóstico para VIH (Incluye en la ejecución, evidencia del trabajo realizado, por ejemplo: fotografía adjuntando consentimiento informado de los usuarios fotografiados). Realiza reporte de datos según formatos y periodicidad definida por el equipo de M&E (05 de cada mes) Llenado de papelería de los expedientes de los usuarios según sea necesario. Actualiza sistemas de información como: REDCap, VICITS Web y EpiWeb; según sea necesario. Realiza solicitud y liquidación de gastos menores de operación para sus actividades relacionadas con las pruebas del VIH y seguimiento de contactos de la estrategia NAC. Anotar las horas que sean dedicadas al proyecto cada día en una hoja denominada Timesheet, misma que deberá enviar en tiempo y forma definido por su supervisor del sitio. 18. Garantiza el correcto manejo y cuidado de los equipos y materiales asignados. Actividad: asegura el uso adecuado del equipo de cómputo, telefonía y papelería asignados para sus actividades. 19. Mantiene la confidencialidad de la información de los participantes. Actividad: Implementar protocolos de resguardo de datos y garantizar la privacidad en la gestión de la información. 20. Mantiene un estricto cumplimiento de las normas de convivencia y de cumplimiento obligatorio en la unidad de salud y con otros compañeros de trabajo del programa VIHCA. Actividades: Se asegura de cumplir con lo establecido en el código de ética de Universidad del Valle de Guatemala. Contribuye con las actividades requeridas por el coordinador de la unidad de salud. Cumplir a cabalidad con las directrices definidas por VIHCA en el desempeño de sus funciones y actividades. Llevar a cabo todas aquellas funciones y actividades requeridas por VIHCA o la unidad de salud que sean relacionadas con su puesto de trabajo. Requisitos Formación: Diploma de nivel medio, idealmente con estudios universitarios/técnicos de Psicología, Enfermería, trabajador social o carreras afín. Experiencia general y específica: Menos de 1 año (de preferencia 2 años) como promotor/navegador u otro puesto similar. Idealmente experiencia en los siguientes temas de VIH: Pre y post consejería para entrega de resultados de VIH. Atención, apoyo emocional y manejo de emociones. Conocimiento del sistema de referencia a Unidades de Atención Integral.
