Duties & Responsibilities: Provide comprehensive assessments and facilitate person centered planning. Provide support and crisis intervention with infants and their families. Assist in coordination and referrals of support services to parents. Provide home-based therapeutic services. Facilitate support groups as needed. Submit timely and accurate paperwork. Maintain client files in order and up to date. Participate in individual and group supervision. Participate in ongoing professional development including but not limited to in-service training, peer review, external workshops and seminars. Attend all agency and department meetings as required.
