Position Summary: The Health Assessment Manager leads MANNA's strategic approach to addressing food insecurity through health-focused interventions and community-level assessment work. Building on the foundation established by the Healthy Opportunities Pilot (HOP) program, this position advances MANNA's understanding of the intersection between food access, nutrition, and health outcomes across Western North Carolina. The Health Assessment Manager designs and implements comprehensive community food needs assessments, develops data-driven strategies to address health-related food insecurity, and builds partnerships with healthcare providers, community organizations, and other stakeholders to create sustainable pathways connecting food security to improved health outcomes. This role serves as MANNA's subject matter expert on food as medicine initiatives and health equity, ensuring that programs and partnerships effectively address the nutritional and health needs of underserved populations throughout the 16-county service area. Essential Duties and Responsibilities Food is Health Strategy & Program Development · Develop and implement MANNA's comprehensive food is health strategy, building on learnings from the HOP program and aligning with organizational goals for addressing root causes of hunger. · Design and lead community food needs assessments across MANNA's 16-county service area, utilizing both qualitative and quantitative research methods to understand the intersection of food insecurity and health outcomes. · Analyze assessment data to identify gaps, opportunities, and priorities for health-focused food interventions, with particular attention to vulnerable populations and communities experiencing health disparities. · Create evidence-based programming that connects food access to health outcomes, including but not limited to fruit and vegetable prescription programs, healthy food box initiatives, and nutrition education integrated with food distribution. · Develop metrics and evaluation frameworks to measure the health impact of MANNA's programs, tracking outcomes such as food security status, dietary quality, and health indicators. · Serve as MANNA's thought leader on food is medicine initiatives, staying current with research, best practices, and emerging trends in the field. Partnership Development & Healthcare Integration · Build and maintain strategic partnerships with healthcare providers, health systems, managed care organizations, and public health agencies to integrate food security screening and interventions into healthcare settings. · Collaborate with the HOP team during program transition to ensure knowledge transfer, maintain key relationships, and identify opportunities for continuing successful elements of the program. · Work closely with the Mountain Community Insights (MCI) team to integrate health-related questions into intake processes and utilize MCI data to inform food is health strategies. · Partner with the Benefits Outreach team to connect food is health participants to SNAP and other nutrition assistance programs, recognizing these as critical tools for improving food security and health. · Engage community partners, including partner agencies and community markets, to incorporate health-focused approaches into food distribution and to serve as sites for health-related assessments and interventions. · Represent MANNA at community health coalitions, food policy councils, and other relevant forums, positioning the organization as a leader in addressing food insecurity as a social determinant of health. Assessment & Research Leadership · Lead the design and implementation of comprehensive community food needs assessments, including survey development, data collection planning, community engagement strategies, and analysis protocols. · Ensure assessment methodologies are trauma-informed, culturally responsive, and accessible to diverse populations, including those who face language barriers, transportation challenges, or other obstacles to participation. · Coordinate cross-departmental involvement in assessment work, engaging Agency Relations, Community Markets, Youth Programs, Nutrition, and other teams as appropriate. · Analyze assessment findings to identify priority areas for intervention, gaps in service delivery, and opportunities for innovation in addressing food insecurity and its health impacts. · Translate assessment findings into actionable recommendations for program development, service delivery improvements, and resource allocation. · Prepare reports, presentations, and other materials to communicate assessment findings to internal and external stakeholders, including staff, board members, funders, policymakers, and community partners. · Utilize assessment data to support grant applications, advocacy efforts, and strategic planning processes. Collaboration & Cross-Functional Integration · Collaborate with Agency Relations and Community Markets teams to identify opportunities for integrating health screenings, nutrition education, and health-related resource connections into food distribution sites. · Support the development of community hubs that provide wraparound services, ensuring that health-related needs are addressed alongside food access. · Contribute to MANNA's Service Area Assessment and Operational Response Plan response by providing health-focused data and recommendations for gap areas. · Participate in MANNA's strategic planning processes, offering expertise on food is health trends, best practices, and opportunities for innovation. Grant Management & Reporting · In partnership with MANNA's Development team, identify and pursue grant funding opportunities related to food is health initiatives and community assessment work. · Develop grant proposals, including program descriptions, budgets, timelines, and evaluation plans. · Manage grant-funded projects, ensuring compliance with funder requirements and timely completion of deliverables. · Prepare and submit accurate and compelling grant reports that demonstrate program impact and outcomes. · Maintain detailed records of program activities, participant outcomes, and expenditures to support grant reporting and program evaluation. Team Leadership · Provide leadership, supervision, and professional development support to any direct reports, contractors, interns, or volunteers working on food is health initiatives or assessment projects. · Foster a culture of learning, collaboration, and continuous improvement within the food is health program area. · Ensure that staff and volunteers receive appropriate training on health-related topics, trauma-informed care, cultural competency, and other relevant areas. · Support team integration and cross-functional collaboration across MANNA's Community Impact department. Requirements Qualifications: · Education: Bachelor's degree in public health, nutrition, social work, community health, or a related field required. Master's degree in public health, health administration, or related field strongly preferred. · Experience: Minimum of 3-5 years of experience in public health, community health, health promotion, or related field, with at least 2 years in a leadership or management role. Experience with food is medicine programs, food insecurity interventions, or social determinants of health initiatives strongly preferred. · Research & Assessment Skills: Demonstrated experience designing and implementing community needs assessments, surveys, or other research projects.
