Responsibilities Supports a patient’s Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise. Performs patient assessments and collaborates with the multidisciplinary care team to develop and implement a plan of care. Contribute to the continuity of care through effective communication and partnership with the designated Care Team. Supports patients by providing housekeeping or laundry services, shopping or other household tasks, and preparing and serving meals and snacks Provides personal services as needed by the client, such as bathing, dressing, and grooming Helps family members care for their loved one by teaching appropriate ways to lift, turn, and re-position the patient Interface with family members to field concerns and questions, and educate them on the patient’s treatment and aftercare needs Records patient information via our EMR system and notifies branch leadership of any changes or unusual conditions Enhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to our accomplishments Performs on-call responsibilities and on-call services to patients/families as assigned with a minimum of 10 hours a week which may include nights, weekend and holidays.
