Position: Registered Nurse
Department: Medical Clinic
Status: Permanent part-time
Schedule: 28 hours per week (0.8FTE)
Wage: RN ($40.40-$49.37); RN(C) ($48.48-$51.51)
Established in 1969, REACH is a non-profit, community-governed organization with a history of primary care innovation and a commitment to equality and social justice in East Vancouver. We have a multidisciplinary approach to care within our medical, dental, pharmacy, multi-cultural family centre and an Urgent Primary Care Centre (UPCC).

SUMMARY OF POSITION:

The primary care nurse cares for families and individuals across the lifespan. This includes health promotion, screening and assessment, counselling and education, and treatment and for minor and acute illnesses, and chronic care management within the nursing scope of practice. The primary care nurse works independently within the standards, limits, and conditions determined by relevant legislation and BCCNM, and the limits of the nurse’s individual expertise. The nurse is expected to consult with a MRP when appropriate. It is important that the primary care nurse be willing and interested in working collaboratively as part of the interdisciplinary team at REACH; be committed to the goal of improving the health of people living in the community; and adhere to the REACH code of ethics. The nurse reports to the operations manager and is to follow protocols and procedures as set out by the operations manager. The nurse will always act within their scope of practice as set out by the BCCNM.

Responsibilities:

  • Assessment and treatment of specific acute and minor illnesses in adults and children in accordance with BCCNM guidelines and decision support tools within individual scope of practice.
  • With BCCNM certification: contraception counseling, treatment and follow-up.
  • With BCCNM certification: assessment, diagnosis and treatment of sexually transmitted infections.
  • Well-child care including assessment of growth and development, and family function.
  • Preventative care including: cancer screening, chronic disease management, harm reduction services.
  • Preventative care such as adult and childhood immunizations, basic nutritional counseling and smoking cessation, support for healthy lifestyle modifications, and assessment of readiness for change.
  • Screening for diseases such as diabetes, hypertension, addiction.
  • Perinatal care, including chest/breast feeding support, prenatal assessment and support, postpartum assessment and support, pregnancy testing and, pregnancy counseling (to include referral to abortion services if requested).
  • Prioritization, case-management and scheduling of walk-in patients during drop-in hours, or phone calls at any time, as the Nurse of the Day (NOD).
  • Limited supportive counselling for patients in crisis; referrals as needed to appropriate community resources.
  • Advocacy for patients as needed.
  • Referral to community programs and/or support services as needed.
  • Consult with a MRP at any point in the patient’s care when the nurse approaches their limits of scope of practice or competence.
  • Appropriate recording in electronic medical records and other clinical reports as required.
  • Willingness to teach nursing or medical trainees as appropriate within the individual’s scope of practice.
  • Pro-active participation in program development and regular professional development

QUALIFICATIONS AND SKILLS:

  • Completion of a recognized Registered Nurse education program and current registration to practice in BC.
  • BCCNM certified practice in Reproductive Health, or willingness to obtain certification
  • BCCNM Remote Practice certification is desirable.
  • Adult and child immunization competency as per BCCDC.
  • Current HCP First Aid Certification.
  • Significant nursing experience (minimum five years) in a variety of settings.
  • Ability to work both independently and collaboratively within an interdisciplinary team.
  • Experience in Public and/or Community Health Nursing.
  • Experience with mental health and addictions an asset.
  • Experience working with marginalized populations (mental health and addictions, poverty, vulnerable youth, Indigenous persons and minorities, LGBTQ2IA+, and other community members)
  • Ability to communicate effectively with patients, co-workers and community.
  • Ability to work in a respectful way with a wide range of patients.
  • Commitment to the concept of the Community Health Centre as an effective means of achieving comprehensive health care.
  • Ability to speak a second language is an asset
