Description AVAILABLE: Medication Assisted Treatment (MAT) LPN - Part Time Days MAT nurse can be 8a-12p 16 hours a week so last time it was M-Thurs PrimeCare Medical, Inc. is looking for dedicated and hard-working Licensed Practical Nurses (LPN) to work in the medical department at the Kent County Detention Center, located in Chestertown, MD. In this role, you will provide comprehensive nursing care and perform duties under supervision of an RN. JOB RESPONSIBILITIES: The Licensed Practical Nurse will primarily be responsible for providing comprehensive nursing care while ensuring the proper flow of relevant information is communicated to appropriate personnel. In addition, the Licensed Practical Nurse will also maintain, update and work with electronic medical records and ensure that they are secure. Other responsibilities will include: GENERAL DUTIES: A. Deliver nursing care for Medication Assisted Treatment Program in accordance with PCM Policies and Procedures. Ensures that nursing practice complies with established nursing standards, and scope of practice limitations. B. Function as MAT Nurse and coordinate care for all patients receiving medications for substance use disorder. C. Provide comprehensive nursing care while observing and maintaining security requirements. D. Performs duties under supervision of an RN pursuant to protocols established by the Medical Director. RESPONSIBILITIES: A. Work collaboratively with multidisciplinary team of PCM nurses, providers, security, administration, and community providers. B. Responsible for coordination and implementation of daily dosing of MAT medications in accordance with facility procedures and PCM dosing policy. C. Maintain a professional wkg. relationship with facility staff, unit staff, providers, and corporate staff. D. Act as MAT patient liaison through permitted communications on behalf of PCM, the contracted facility and outside agencies. E. Coordinate care for all MAT Patients and/or applicable candidates by obtaining records, coordinating care and making appropriate referrals. F. Ensure compliance with established standards of PCM. Follow established Corporate and facility policies and procedures for MAT in clinical practice. G. Ensure that the proper flow of relevant information (inmate/patient information, unit problems) is disseminated to appropriate personnel. H. Maintain security / privacy of all medical records. Maintain documentation and communicate in accordance with HIPPA and 42 CFR guidelines for confidentiality. I. Monitor nursing care to avoid the inappropriate use and/or duplication of services. Advises HSA and charge nurse if these situations arise. J. Screen for appropriate referrals to providers for MAT, ensure all follow up tasks are created. K. Ensure strict compliance to all applicable pharmacy regulations for ordering, storing and administration of Medications for Opioid Use Disorder and Alcohol Use Disorder. L. Monitor stock inventories of supplies and equipment and alerts head nurse of needs. M. Obtain regular lab specimens as required by MAT program policy. N. Assist patients with community reentry by linking to community MAT providers, obtaining ROIs and faxing all pertinent records. O. Attend scheduled Interdisciplinary Team Meetings for MAT weekly. Keep recorded minutes. P. Report data as required and participate in program evaluation. Maintain on going records pertaining to MAT patients for quarterly and yearly statistics. Q. Attend seminars, workshops, conferences at the direction of PCM. R. Flexibility in work schedule is required. Unpredictable circumstances may warrant additional work hours and overnight travel to the facilities. S. May be required to relocate (temporarily) to meet the needs of PCM. T. Adhere to established PCM and facility policies and procedures. U. In the absence of the charge nurse, is operationally responsible and accountable for the nursing functions of the medical unit. V. Provides other services and performs other duties as assigned by charge nurse, HSA, or Corporate staff. W. Maintains accurate records of all controlled substances, narcotics, and sharps in accordance with applicable state and federal laws. X. If necessary - Conduct sick call (if permitted by state regulations) for inmate/patients and provide treatment for selected minor illnesses and injuries, defined by written protocol and approved by PCM Medical Director. Refer all cases not covered by written protocol or that do not respond to treatment to an appropriate PCM physician. Y. Receive and give inter-shift report to ensure continuity of care and to disseminate pertinent information necessary to the functioning of the oncoming shift. Z. Observe, report, and document symptoms, reactions and progress of inmates/patients and participate in the formulation of the plan of MAT care. This position provides a 2-week orientation period where you will be training on-the-job and shadowing with the nursing staff.
