QUALIFICATIONS:

• Graduate of an accredited school of nursing – required.
• Current license to practice as a registered nurse in the state of Massachusetts – required.
• Current CPR – required.
• One year nursing experience, preferably med-surg - preferred
• Possess a valid State of Residence Driver’s license and reliable transportation – required.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:

• Assesses patient’s health problems and needs and develops plan of care.
• Provides direct skilled nursing care as per physician orders.
• Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
• Document medical records and prepare other reports as needed.
• Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.

SUPERVISORY RESPONSIBILITIES

• Completes home health aide supervisions and orientations as needed.
• Co-signs orders and provides oversight of care for LPNs.
