Responsibilities Contributes toward accomplishment of patient’s Plan of Care goals by providing mobility activities as assigned by the nurse. Provides assistance to persons served and their family to learn to safely manage personal care while increasing activity, strength, and endurance. Utilizes a holistic approach (considering physical, psych/social, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service area. Demonstrates initiative to improve quality and customer service. Promotes professional rehabilitation/mobility objectives among interdisciplinary team members Participates in process improvement for enhancement of care delivery and clinical excellence Demonstrates competence in the implementation/administration of age-appropriate care in accordance with established Practice Guidelines, Policies/Procedures and State/Federal Law in regard to hygiene, nutritional, elimination, and activity measures as well as non-sterile/non-invasive procedures, oxygen setup, simple dressings, traction set-up, blood glucose testing, measuring and recording vital signs and non-sterile specimen collection. Promptly completes documentation to assure an accurate legal record of patient’s care. Qualifications Education: High School diploma or equivalent is required. Certifications: Basic Life Support (BLS) within 90 days of hire. Basic Skills and Abilities: Basic computer knowledge and the ability to document accurately and timely Ability to deal efficiently and effectively with a varied patient population and the health care team Effectively communicates with all team members, physicians, ancillary staff and all customers.
