The Opportunity In this full-time, on-site role based in Newton, Massachusetts, the Case Manager is a key member of Asteroid Health’s interdisciplinary treatment team. The Case Manager provides direct care coordination, case management, and client advocacy across Asteroid Health’s continuum of mental health services. This position works closely with clinicians, prescribers, and program leadership to support treatment planning, facilitate referrals, coordinate services, and ensure continuity of care. The Case Manager plays an essential role in supporting client engagement, monitoring progress toward treatment goals, and maintaining compliance with clinical documentation and regulatory standards.
