Case Management Officers are responsible for delivering evidence‑based case management to a diverse caseload of offenders, ensuring their individual risks and needs are addressed to maximise rehabilitation opportunities and reduce reoffending.   What you’ll do As a Case Management Officer, you’ll play a vital role in transforming lives and creating safer communities. Your day-to-day work will be dynamic and impactful. As a valued member of our dedicated Service Integration team, you will: Deliver high‑quality, evidence‑based case management services to offenders, ensuring all activities align with relevant legislation, policies and guidelines. Conduct comprehensive assessments to identify risks, needs and strengths, using these to develop personalised case plans that support offenders through their rehabilitation journey. Apply motivational interviewing and strengths‑based approaches to support behavioural change and encourage offender accountability. Work collaboratively with internal and external service providers to connect offenders with programs and interventions that best align with their individual case plans. Monitor progress against case plans, ensuring compliance with judicial and departmental requirements, and preparing timely reports and documentation. Build and maintain strong relationships with stakeholders to support effective reintegration planning and assist offenders in making a successful transition back into the community. What we’re looking for We’re seeking an individual who brings: Demonstrated experience delivering evidence‑based interventions to diverse offender groups, including those with complex needs. Proficiency in using risk assessments and case management tools to design targeted, outcomes‑focused intervention plans. Strong analytical and problem‑solving skills, with experience addressing challenging behaviours using proven behavioural intervention strategies. Sound understanding of contemporary correctional practices, with a commitment to offender rehabilitation and reducing reoffending. Experience working effectively within multidisciplinary teams to provide cohesive, person‑centred case management support. A current Certificate IV in Correctional Practice with the relevant Units of Competency or the ability to complete the qualification through the Corrective Services Academy within required timeframes.
