In this role, you will provide case management and personalized support to individuals with disabilities and their families. You’ll help clients manage their FMS budgets, navigate Minnesota waiver programs, coordinate services, and ensure their funding aligns with their goals and priorities. As a CDCS Service Coordinator, you’ll work with a high level of autonomy while being part of a collaborative Self-Directed Services team.
