Essential Duties and Responsibilities Provide support to persons served. Teach each person served the skills needed to accomplish goals set in their Individual Service Plan (ISP). Document outcomes of ISP. Document facility safety practices, assessments, incident reports and daily logs. Supports persons served in development of connections/relationships in the community. Attend ISP meetings as requested. Attend monthly persons served Self Government meetings. Guide persons served in making responsible choices. Transports persons served as needed. Inform Program Coordinator of issues experienced by person served in ISP implementation. Supervisory Responsibilities None Minimum Qualifications High school diploma or equivalent education level. 18 years or older. Must be able to obtain and maintain certification in CPR/First Aid. Must be able to obtain and maintain Blood Borne Pathogens, Medication Administration, Systematic Instruction Strategies, Abuse/Neglect, HIPAA, Confidentiality, and Positive Behavior training.
