The LSC monitors and provides care coordination and life skills training and development, in collaboration with the care team, to youth and/or adults with co-occurring mental health and/or substance use disorders- including those with serious and persistent mental illness. Ability to help clients regulate emotions. Document services provided in Electronic Medical Record (EMR) based on policies and procedures.
