The Community Health Worker goal is to facilitate enrollments of LVHC clients in available health insurance programs and public benefits.
We are seeking a full-time Community Health Worker to work with our health clinic team and provide comprehensive services to community members.

SKILLS AND QUALIFICATIONS:

• Experience in the medical field preferred.
• Documentation skills
• Compassion, especially when clients are experiencing complex issues
• Exceptional written and oral communication skills, to present a care plan to clients so they understand fully
• Exceptional listening skills, to understand what clients need and develop a plan that will work for them
• Organizational skills, managing all the moving parts of client treatment planning and care
• Cultural competence, to understand and value clients’ unique perspectives
• Experience in human services including working with individuals who use substances, individuals who are homeless, or individuals with complex medical conditions highly preferred.

PHYSICAL REQUIREMENTS:

1. Must be able to lift up to 25 pounds.
2. Must be able to hear staff on the phone and those who are served in person and speak clearly in order to communicate information to clients and staff.
3. Must have vision which is adequate to read written materials, computer screens, registration forms and other clinic documents.
4. Must have manual dexterity and sensitivity sufficient to perform routine procedures as required.
5. Must be able to reach above the shoulder level to retrieve small items from storage; must be able to bend, sit, stand, crouch, reach, twist, and turn.
   WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually average.

• The development of collaborative care management plans with clients which support clients’ needs in the areas of physical health, mental health, substance use disorders, community-based long-term services support, oral health, palliative care, social supports, and social determinants of health.
• Core Community Health Worker activities include but are not limited to, outreach, comprehensive assessment and care management, care coordination, health promotion, comprehensive transitional care, identifying client support needs, and coordination of and referral to community and social services support.
• General case management for supporting all at Long Valley Health Center including and not limited to enrolling patients in federal and state provided benefit programs, coordinating dental appointments to navigate barriers to care, assisting with education and compliance of dental treatment plans or interventions, communicating with PCP’s and/or other specialists, and coordinating with community agencies as appropriate.
• Documentation and Billing of services rendered
• Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
• Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
• Chronic care management- supporting patients with 2 or more problem on their problem list (chronic conditions) with creating care plans and establishing goals with patients with Medicare and Partnership and coordinating their medical care and referrals
• Coordinating and planning hospital discharge to home or other care setting: Assisting with all discharge needs until a smooth transition has been established.
• Outreach including groups that are partnership QIP eligible
• Knowledge of the tasks and/or prior experience as a dental assistant is highly preferred.
• Knowledge and understanding of medical terminology, medical coding, insurances, and treatment plans.
• Problem solving/critical thinking skills are required-must be independent and able to handle tasks and challenges.
• Anticipate and be proactive in addressing the barriers (psychological, financial, medical, transportation, etc.) of accessing dental care.
• Must have outstanding written and verbal communication, display compassion with patients, their family members, caregivers, and staff.
• Lead educational campaigns in the schools and surrounding communities on the importance of oral health and its relationship to systemic health.
• Present and discuss treatment plans. Assist in prioritizing treatment plan and establish a plan for scheduling and patient compliance.
• Build trust and understanding of patient’s goals and situation.
• Uphold all LVHC organizational policies.
• Must be self-motivated, proactive, and organized while working in a team setting.
• Encourage and educate patients in completing necessary dental treatment and address concerns relating to expenses and anxiety towards medical care.
• Explaining medical terminology and treatment plans to patients.
• Develop innovative marketing and patient outreach plans to increase patient volume.
• Build referral relationships with pediatricians and behavioral health professionals in the community.
