About the role:


  • This is a Permanent Full-Time opportunity offering flexibility and a great work-life balance.Flexible work hours: 8:30am - 4:30pm or 9:00am - 5:00pm, Monday to Friday
  • Attractive remuneration of $45.56 per hour plus superannuation
  • Hybrid work model with 2 days working from home and 3 days in the Queanbeyan office
  • Supporting SAH customers.
  • Key Accountabilities:Build trusted relationships with customers and support them to achieve their goals at home.
  • Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.
  • Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.
  • Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.
  • Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.
  • Recognise and escalate customer risks, safeguarding concerns and changing care needs through appropriate pathways.
  • Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.
  • Promote safety, quality and person-centred care across all aspects of service delivery.

What You'll Bring:

  • Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.Strong customer focus, with the ability to build trusted relationships and navigate sensitive conversations with empathy and professionalism.
  • Excellent communication skills and confidence using Microsoft Office and customer management systems.
  • Knowledge of funding streams, quality standards, audit requirements and care transitions, including discharge planning.
  • Strong decision-making, problem-solving, time management, and prioritisation skills in a fast-paced environment.
  • A collaborative mindset, with the ability to work effectively across multidisciplinary teams, families, clinicians and external providers.
