Ultimately, your contributions will directly impact the quality of life and health outcomes of the individuals entrusted to our care. Minimum Qualifications: * Current and valid Licensed Vocational Nurse (LVN) or RN license in the state of Texas. * Completion of an accredited vocational nursing program. * Basic Life Support (BLS) certification. * Demonstrated ability to perform nursing duties in a long-term care or rehabilitation setting. * Strong communication and interpersonal skills. Preferred Qualifications: * Experience working in a nursing home or rehabilitation facility. * Additional certifications such as IV therapy or wound care. * Familiarity with electronic health record (EHR) systems. * Training in geriatric nursing or chronic disease management. * Ability to speak multiple languages relevant to the resident population. Responsibilities: * Provide direct nursing care to residents, including administering medications and treatments as prescribed. * Monitor and record vital signs, symptoms, and changes in patient conditions, reporting significant findings to the healthcare team. * Assist residents with activities of daily living such as bathing, dressing, and mobility support. * Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans. * Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements. * Educate residents and their families on health maintenance and disease prevention strategies. * Ensure a safe, clean, and comfortable environment for residents by adhering to infection control and safety protocols.
