Essential Functions
Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models
Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence- based practice model and DCCCA organizational policies
Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3- 6 months of referral
Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.

Travel
