Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in–home and school-based services, including in–home and school-based programs to children, youth, families within the community.
Lynwood Charlton Centre is the Lead Agency for the Hamilton service area for Moving on Mental Health. This is a Ministry initiated plan to transform the experiences of children, youth and families dealing with mental health issues, and strengthen the community-based system for delivering mental health services to them. Through our Lead Agency role, we are responsible for leading and facilitating system reform in Hamilton. To do this, we work collaboratively with other child and youth mental health service providers, other affiliated community partners including our child welfare, health, and school board partners, and parents and youth engaged in our services.
The Centre is funded and licensed by the Ministry of Health (MOH) and the Ministry of Children, Community and Social Services (MCCSS) and accredited by the Canadian Centre for Accreditation.

KNOWLEDGE REQUIRED
· Knowledge of child and youth mental health and relevant evidence informed treatment interventions
EDUCATION AND TRAINING QUALIFICATION(S)
· Diploma in CYC or Equivalent from recognized community college or a BA in a related field
· First Aid

ROLE OVERVIEW
Working under the direction of the Manager of Day Treatment Services , the Day Treatment Child and Youth Care Practitioner is responsible for the provision of day-to-day care and utilizing best practice and evidence informed treatment interventions with children/youth in a classroom environment. The Day Treatment Child and Youth Care Practitioner will perform prime worker/coach duties through developing effective relationships with caregivers/guardians and service providers in the community.
RESPONSIBILITIES
· Providing a safe, predictable, and therapeutic environment for the youth in the day treatment program by :
· Providing treatment to youth and families
· Provision of information and education to youth, families and community collaterals to support the implementation of the treatment plan
· Providing individual counselling to youth and their families
· Case Management of youth and families engaged in the Day treatment program
· Attend and participate in team meetings
· Contribute to the development, implementation and evaluation of the Compass Day Treatment program.
· Contribute to effective team development and processes to promote team cohesion and communication including cross program collaboration
· Contribute to the development of the field by supervising students on placement
· Other duties as assigned.
