Position Summary Holon Health is seeking an experienced and compassionate Nurse Practitioner licensed and located in Colorado to support individuals with SUD through collaborative, integrated care. This role combines telehealth-based care with periodic in-person presence across our Colorado locations (Denver, Fort Collins, Colorado Springs, and Basalt) to support our hybrid Care Navigator, Case Manager, and Peer Recovery Specialist teams. Travel frequency will vary based on client and team needs—we're looking for someone who values flexibility and is energized by both virtual and face-to-face connection. This position offers remote flexibility and is structured as a full-time (40 hours/week) role. You’ll provide telehealth-based assessments, medication management, and care coordination to support clients on their recovery journey—and be part of a forward-thinking team breaking down barriers to behavioral healthcare.
