They collaborate with clients to understand their goals and needs in order to tailor individualized plans that are preventative and restorative in nature. Main Job Duties Providing high quality, skilled, whole-health services within a rehabilitative and therapeutic model of care Actively engaging with clients, developing positive relationships that support the client in meeting their goals. Acting as a collaborative team member to any program within the organization, leading and addressing comprehensive health promotion. Facilitating preventative and restorative health and well-being education in individual, group and community settings. Using experiential learning as an intervention to support recovery. Acting as a liaison between the client and external care providers for whole health needs. Providing information and/or referrals to resources in the community for increased support and stabilization. Offering coaching on medication management. Documenting observable behaviors and symptoms. Requirements Required Qualifications: Current and unrestricted license as a Registered Nurse in the state of Minnesota Excellent driving record, reliable car, and current auto insurance And meet one of the conditions below.
