Jefferson Health at Home by BAYADA is currently seeking an experienced clinician (RN, LPN, OT, OTA, PT, PTA, or Social Worker) to fill the position of Transitional Care Manager (TCM) . Clinical Liaisons at BAYADA interact with discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of home health care (Medicare) services.
This will be a hybrid role. The TCM will work from home, and on-site at Jefferson Einstein Montgomery Hospital in Norristown, PA.

QUALIFICATIONS FOR A TCM:

  • Holds a current license in good standing as a clinician (Nurse, therapist or social worker) in the state of practice OR have experience with case management in a Hospital setting
  • Demonstrated record of strong interpersonal skills.
  • Demonstrated knowledge of the Medicare COPs and other program requirements, as applicable.
  • At least one (1) year experience in health care, preferably home health care.
  • Previous discharge planning experience preferred.
