Job Summary Come work as a Mental Health & Addictions Support Worker with Vancouver Coastal Health (VCH)!Vancouver Coastal Health is looking for a Mental Health & Addictions Support Worker to join the Older Adult Mental Health and Substance Use team in Sechelt, BC. As a Mental Health & Addictions Support Worker with VCH you will: Provide support services to clients who are seriously affected by substance use and/or mental health problems. Monitor clients, documents and provide verbal reports on observations in accordance with standard procedures or specific direction, facilitate the use of community services and resources and provide information on community services, and liaise and coordinate services with mental health, addictions, medical professionals and other related services. Participate in the process of case planning, assist clients in the development of life skills and healthy lifestyles and escort clients to appointments as needed. Participate in team meetings and corporate initiatives, engage in ongoing team planning and quality improvement activities. Participate in maintaining a healthy environment for clients and team, and perform a variety of clerical duties as required. ABOUT SECHELT, BC The District Municipality of Sechelt is located on the lower Sunshine Coast of British Columbia, approximately 50 km northwest of Vancouver.
