OVERVIEW

We are seeking a dedicated and detail-oriented Care Navigator to join our team. The ideal candidate will play a crucial role in supporting clients by guiding them through their care journey, ensuring they receive the necessary services and resources. This position requires strong organisational skills, excellent communication abilities, and proficiency in various administrative tasks. The Care Navigator will work closely with clients, healthcare providers, and community resources to facilitate effective care coordination.
